Checklist
Input Validation & Injection
- Patient name fields — sanitized for XSS (no script tags, no HTML injection, proper encoding on display)
- Genetic data input — validated against expected formats (rsIDs match rs[0-9]+, HGVS notation validated, allele values are valid bases)
- File uploads — file type validated server-side (not just client-side extension check), file size limited, content scanned
- Search queries — user search input parameterized, not concatenated into queries
- Analysis parameters — validated against allowed ranges (age > 0, percentile 0-100, not arbitrary values)
- SQL injection — all database queries use SQLAlchemy ORM or parameterized queries, never string concatenation
- NoSQL injection — if applicable, query operators from user input blocked
- Path traversal — file path parameters validated (no ../ sequences, no absolute paths from user input)
Unauthorized Access (IDOR)
- Patient data isolation — API endpoints verify the requesting user has access to the requested patient's data
- Direct object references — sequential/guessable IDs (patient/1, analysis/2) checked against user's authorized scope
- Horizontal privilege escalation — Patient A cannot access Patient B's genetics by changing the ID in the URL/API call
- Vertical privilege escalation — patients cannot access counselor-only or admin-only endpoints
- Data in responses — API responses don't leak other users' data in metadata, lists, or error messages
- URL parameter tampering — changing query parameters doesn't bypass authorization checks
Rate Limiting & Resource Abuse
- Genetics analysis rate limit — expensive computation endpoints rate-limited per user (prevent CPU abuse)
- API rate limiting — general API rate limits prevent automated scraping
- Auth endpoint protection — login/register endpoints rate-limited to prevent brute force
- File upload limits — upload size and frequency limited per user
- Search rate limiting — search/query endpoints rate-limited to prevent data scraping
- Web Worker abuse — client-side computation can't be abused to exhaust server resources through result storage
XSS Prevention
- Output encoding — all user-supplied data HTML-encoded before rendering
- React auto-escaping — JSX expressions auto-escape; verify no dangerouslySetInnerHTML with user data
- Rich text — if clinical notes support formatting, content sanitized (DOMPurify or similar)
- URL parameters — query parameters reflected in UI are sanitized
- Error messages — user input in error messages is escaped (e.g., "Patient 'name' not found" — is 'name' escaped?)
CSRF & Session Abuse
- State-changing requests — POST/PUT/DELETE protected by CSRF tokens or SameSite cookies
- Session hijacking — cookies use httpOnly, Secure, SameSite flags
- Cross-origin — CORS restricted to trusted origins only
- Clickjacking — X-Frame-Options or CSP frame-ancestors set
Data Enumeration
- User enumeration — login and registration don't reveal whether an email/username exists
- Patient enumeration — sequential IDs don't allow iterating through all patients
- Timing attacks — consistent response time for valid/invalid resources (prevent enumeration via timing)
- Error detail — error messages don't reveal system internals (database type, table names, file paths)
Abuse Monitoring
- Audit logging — access to genetic data logged with user ID, timestamp, and accessed resource
- Anomaly detection — unusual access patterns flagged (counselor accessing hundreds of patients, patient viewing data at 3am)
- Failed auth logging — failed login attempts logged and monitored
- Data export controls — bulk data export limited and audited
