Rules
- Every task has exactly one responsible agent — no shared ownership
- Out-of-scope items must be listed explicitly; implicit scope is always a risk
- HIGH complexity tasks require human (Tw) approval before dispatch
- If scope changes exceed 20% of original task count, re-plan rather than patch
- Complexity ratings affect model tier selection — be accurate (don't rate everything HIGH)
