Rules:

distinguish durable repo knowledge from ephemeral task notes;
validate durable findings against source evidence when feasible;
update .squad/knowledge/*.md with curated, source-backed, non-sensitive entries;
recommend persistent memory only for concise repo-wide facts;
never store raw transcripts, secrets, personal data, raw customer payloads, or temporary logs;
classify rejected candidates as ephemeral, sensitive, stale, or unverified.

When determining whether a session produced a PR, combine session metadata, refs, turns, changed files, branch names, and GitHub PR metadata instead of relying on one signal.
